    Attending the IFSO World Congress in Naples from August 29th to September 1st provided me a unique opportunity to immerse myself in a significant event featuring numerous activities with world-renowned specialists. Many of these experts are highly regarded for their academic contributions in bariatric field. This experience proved to be both academically enriching and professionally rewarding. Notably, this marked the second time I attended an IFSO World Congress in person and the third, considering the virtual edition. My strong interest in actively participating in IFSO events has been a driving force behind my engagement. I attended various key sessions early in the day, which included Revisional Bariatric Surgery, Sleeve Gastrectomy I, Nutritional Management, New Obesity Drugs, GERD and Hiatal Hernia, and Swallowable and Intragastric Balloon. The first day of presentations provided a remarkable opportunity to witness different perspectives and research approaches on the same topics from various regions around the world, as the presenters hailed from America, Asia, the Middle East, Europe, and Oceania. The depth of discussion often reached a high academic and experiential level due to the large patient volumes managed in other countries. Experiencing live presentations in this diverse environment was truly gratifying. During these sessions, a very recent topic was discussed: the experience with intragastric balloons and patient follow-up, with multiple surgeons sharing their insights. Among them, I was particularly impressed by the presentation of the Argentine surgeon, Mariano Palermo.

    In the afternoon, the congress officially opened with a warm welcome to the attendees. Dr. Angrisani (Italy) mentioned that the attendance exceeded expectations, with over 3000 participants. Authorities and globally renowned figures in the field of bariatric surgery were introduced as the key speakers. The day concluded with an artistic performance by Stephania Bruno using 'sand art' to narrate the history of Naples, which was enthusiastically applauded by the audience.

    In between sessions, I presented my work in the e-poster panels, where I had the chance to meet esteemed doctors such as Dr. Sergio Aparicio and Dr. Stuardo Behrens, who collaborate closely with Dr. Luis Poggi. They kindly approached me to discuss and inquire about the work I presented, as we are pioneering innovative topics in bariatric surgery and the physiology of the gastroesophageal junction in Peru.

    In the following days, aside from participating in sessions and defending the posters presented, I had the unique opportunity to engage in simulation, a field that I was already familiar with due to my university's encouragement of early involvement. As someone deeply interested in pursuing this specialization, encountering state-of-the-art equipment and being able to operate it was an exceptional experience. I successfully executed exercises using the DaVinci robot, conducted an endoscopy, and initiated my practice with laparoscopic instruments.

    Engaging in discussions about other e-posters from diverse research groups opened up opportunities and allowed me to gain insight into how research is conducted in hospitals, research centers, and universities abroad. This experience has enriched my knowledge and provided valuable insights on enhancing scientific productivity at my university.

    Attendance Objectives:

    At CIFCAD (Center for Investigation in Gastrointestinal Physiology and Bariatric Surgery), a newly established research center in Lima, Peru, our primary focus lies in enhancing academic excellence and disseminating our research findings. Our core mission is to build connections and facilitate collaborative efforts, with the ultimate aim of conducting multi-center projects in the fields of gastrointestinal physiology and bariatric surgery.

    Highlights:

    Among all the activities I had the chance to witness, one particularly celebrated event, which took me by surprise as it marked the inauguration of the congress, was the performance by the sand artist Stephania. It was a delight to see her artistic presentation, accompanied by music, while she recounted the history of Naples. Her detailed information blended perfectly with each of her movements, leaving a lasting impression.

    One of the most intriguing sessions was the one on GERD and Bariatric Surgery, where highly innovative topics were discussed, such as the Nissen-Sleeve and, notably, the Toupet-Sleeve. Both of these approaches showed promising results in the case studies presented by various speakers. As a physiology enthusiast, I was particularly interested in learning about these outcomes, as they might be replicable techniques in my country in the future.

    A pleasant surprise was the announcement of the new president of IFSO worldwide, Dr. Prager. I had previously had the opportunity to review his research on manometry in post-bariatric OAGB patients. We are avid followers of doctors conducting research in post-bariatric physiology.

    Presentations

    As the lead author and head of the Research Center, we presented multiple works, including 4 posters and 2 oral presentations. Our works primarily consist of findings from pre-operative and post-operative studies in bariatric patients. During our oral presentations, the discussions with fellow speakers and panelists proved to be highly enriching. Their perspectives and expertise provided us with valuable feedback, enabling us to prepare these works for publication.

    Our research carries significant academic relevance, evident in the active engagement of the audience and their frequent inquiries. We place a strong emphasis on presenting our materials and videos with the utmost quality and precision.

    I would suggest to have the chance to connect with other scholarship recipients before the congress began, possibly through a conference chat (whatsapp or IFSO app). Such interactions would have fostered the sharing of experiences and the establishment of connections in advance of the conference's commencement.

    I extend my heartfelt appreciation to IFSO and the congress organizers for providing us with a gratifying and culturally enlightening experience. This unique opportunity allowed us to explore a country with a rich historical heritage.

    Furthermore, I wish to express our gratitude for the exceptional support we received from the IT team. Their flexibility allowed us to make necessary adjustments to our posters, and their assistance facilitated the seamless submission of our presentations for the exhibition.

    Attending the XXVI World Congress of the International Federation for the Surgery of Obesity and Metabolic Disorders (IFSO) in Naples from August 30 to September 1, 2023, was an astounding and unparalleled experience. This unique opportunity, made possible by a scholarship, left an indelible mark on my early surgical career in this field.

    One of the most cherished aspects of this event was the incredible networking opportunities. The chance to connect with amazing surgeons in the field, exchange ideas, and build valuable professional relationships was truly invaluable. The constant evolution in the field of metabolic and bariatric surgery, which I was privileged to witness at the congress, is a testament to the dynamic nature of this domain. The rapid technical advancements and scientific breakthroughs in this medical science were inspiring for contributing to this evolving landscape.

    Yet, despite the wealth of knowledge, there was one challenge that highlighted the overwhelming nature of the event: the sheer volume of simultaneous sessions. With an array of fascinating presentations and discussions happening concurrently, it was a real challenge to absorb all the information and insights that I had hoped to. The desire to experience everything clashed with the constraints of time.

    In summary, the XXVI IFSO World Congress in Naples was an extraordinary experience, thanks to the scholarship that made it all possible. It opened doors to a world of knowledge and networking opportunities in the ever-evolving field of metabolic and bariatric surgery. The event left an indelible mark on my medical career, offering a glimpse into the future of this dynamic field in which I pretend to grow.

    I want to express my sincere gratitude for the invitation and scholarship to attend the recent IFSO 2023 congress in Naples‐an experience that I found to be truly enlightening. The diverse array of sessions and the wisdom shared, offered a comprehensive review of current trends and innovations in the metabolic and bariatric surgery field. Personally, engaging in interactive workshops was a true highlight, providing not only a hands-on learning experience but also fostering meaningful connections with fellow attendees. Additionally, the networking dinner events further enhanced the overall experience, providing a relaxed setting for exchanging ideas with colleagues and experts in the field.

    Reflecting on this experience, I offer a few suggestions for future congresses. Firstly, incorporating even more interactive sessions could further deepen participant engagement. Secondly, diversifying session formats, such as panel discussions and roundtables, could provide a more varied and dynamic learning experience. Lastly, exploring options for virtual participation would undoubtedly broaden accessibility and participation. In conclusion, IFSO 2023 served as a valuable platform for both learning and networking, and I eagerly anticipate future invitations to contribute further and connect with our peers.

    Integrated Health Scholarship Report

    Receiving a IFSO Integrated Health Scholarship allowed me to attend and participate in the 2023 Word Congress in Napoli, Italy. This was a tremendous learning and professional development opportunity, as described below.

    While at the conference, I was able to present on my research through two talks: “Changes in 24-hour movement behaviors after metabolic and bariatric surgery and their associations with weight loss: An actigraphy study” and “Promoting physical activity after bariatric surgery: worth the weight?,” presented in the “What I learned from patients that changed our practice” symposium (Chair: A. Schroeder). These talks allowed me to share my research findings with others in the metabolic and bariatric surgery community and to continue to hone my professional presentation skills. I also greatly expanded my knowledge of numerous aspects of metabolic and bariatric surgery through attending scientific sessions focused on topics ranging from the physiological underlying obesity to multidisciplinary approaches to the management of metabolic and bariatric surgery patients.

    In addition to these learning opportunities, I had the pleasure of meeting and networking with professionals from around the globe. I met countless new individuals with whom I hope to stay in contact with over the coming years, a couple of which could turn into productive research collaborations. I also met the editor-in-chief of a metabolic and bariatric surgery-focused journal and, though this connection, have now become an Associate Editor at the journal.

    As a first-time attendee at an international conference, my favorite thing about attending IFSO was learning from professionals from other countries. It was fascinating to hear how the culture around obesity, as well as care for metabolic and bariatric surgery patients, can vary from country to country. For example, in the U.S., there is not universal healthcare, so patients’ health insurance access and benefits can vary widely. It was therefore very interesting to attend presentations from individuals in countries that have national healthcare and to hear how care differs in those locations. I also really enjoyed the many networking opportunities offered at IFSO. This included the lunches provided each day and the evening social events. In addition to being enjoyable, these were wonderful opportunities to meet and connect with new colleagues.

    In terms of improvement, given that I am an early career professional, it could be beneficial to offer programming that specifically aims to foster relationships between early career and more established researchers and practitioners. For example, at some conferences that I have attended, they offer coffee hours geared toward this purpose or even arrange 1:1 mentor-mentee relationships ahead of the conference and then have a time for these pairs to meet briefly in person at the conference.

    Overall, attending IFSO was a fantastic personal and professional experience, and I am so thankful to IFSO for providing me with an integrated health scholarship to facilitate my participation at this conference. I look forward to attending another IFSO conference in the near future.

    My experience in the International Bariatric Congress in Naples 2023 was very enjoyable. I got to watch many great presentations from great surgeons all over the world. Also, I gave my presentation in front of a full room and it was one of the best academic experiences I´ve ever had.

    I was pleased to meet and talk with lot of surgeons from countries very far from mine and it was amazing to learn how they work and operate in their countries. My favorite presentations were the video format presentations, because I could see other techniques and discuss the details.

    In conclusion, it was an amazing experience and I´m very grateful. I´m very honored of the opportunity that IFSO gave me to present my work. I´m very proud of this congress and all the organization behind it. I would really like to be present next year.

    Thank you to all IFSO organising and the scientific committee for such a great conference. I was humbled to receive the Integrated Health Scholarship Award and be able to attend this year’s IFSO in beautiful Naples. It was also a privilege to have my paper accepted for an oral presentation and such a great opportunity to network with the leaders in this field and share knowledge across the globe.

    IFSO this year was held in Napoli with a fantastic program, including 4 days of conference with 8 concurrent sessions and several Post Graduate courses. There were over 3000 delegates from 150 countries, bringing so much energy and enthusiasm to share knowledge. It was an amazing conference as always, with a great program, well organised and with presentations from leaders in the field of metabolic and bariatric surgery (MBS).

    The highlights this year where presentations by Dr. F Rubino, Dr. R Batterham, Dr Carel Le Roux, Dr Scott Shikora and Prof. Angrisani. Highlighting the focus on the neuroendocrine hormones, the current treatment for the condition of obesity and the safety and efficacy of metabolic surgery.

    The original bariatric care management and surgical guidelines were published by NIH in 1991. Since then the procedures, the techniques and the overall understanding of obesity as a chronic condition and its best management has changes. Hence, the IFSO/ASMBS associations reviewed and updated these much overdue guidelines. It was fantastic to hear the presentation by Dr. Scott Shikora (President of IFSO) and the multidisciplinary team on the development and ways to implement these guidelines in practice.

    I also particularly enjoyed the integrated health PG workshop, hearing a great range of topics and presentations on managing medical, psychological and nutritional complications following MBS. Speakers such as Violeta Moiz, Mary O’Kane, Julie Parrott highlighted how important it is to not only screen and treat nutritional deficiencies early but also ensure that there is continued monitoring until the symptoms have resolved.

    The presentation from Mr. Ken Clare (lived experience and RN with 45 years) was also great. Ken was very inspirational and through sharing his journey, highlighted how important it is to focus on what patients need. It is important to always have our patient at the centre of care, identify the gaps in what patients need and what us as HCP cover with them in our education sessions.

    The presentations from Eugena Romano, Eva Conceicao and Valentina Ivezaj, on the psychological health care of patients, working through challenges on eat the an addressing the stigma were fascinating. Learning about the presence of mental health disorders in particular Trauma and eating disorders in our pre MBS patients. Identifying this condition, working through them with our clients and referring on to appropriate health professional to enhance care was highlighted to be the key.

    The take home message was addressing stigma, using appropriate language, empowering our patients and placing them in the centre of our care as well as working within a Multidisciplinary / Integrated health / Transdisciplinary care team is essential in our management of patients throughout their MBS journey.

    What I would like more of in the future conferences is hearing more on

    • Nutritional complication prevention and management, such as metabolic and bone health disease.
    • Practical ways
    • How to look after ourselves as health professionals, in order to avoid compassion fatigue, burnout etc., in order to be able to look after our patients better
    • Hearing from our lived experiences with the aim to bridging the gap and addressing their needs better
    • Case studies and practical approaches in managing medically / psychologically complicated cases

    I am really excited and looking forward to the IFSO in Melbourne, Australia next year and hope to see many of my colleagues there! Thank you again to the IFSO team for such an amazing conference and supportive scientific community. Looking forward to be able to continue contributing and learning from you.

    I express profound gratitude to the members of IFSO for granting me the opportunity to participate in this enlightening event. Not only did I have the privilege of being an observer, but I also actively contributed my insights. The entire conference was a transformative learning experience that left me with a more comprehensive and enlightened perspective on the challenges faced by Bariatric patients.

    The conference shed light on several critical aspects, including the stigmas associated with weight, issues related to body image, and the prevalent misconceptions within the field. It also emphasized the paramount importance of psychological community support and its profound influence on the well-being of Bariatric patients.

    One of the key takeaways from the conference was the knowledge I acquired about effectively addressing the multifaceted problems encountered by obese and Bariatric patients. Specifically, I developed a deeper understanding of the impact of food addiction on weight gain, sparking my interest in the field of food de-addiction. I am now motivated to develop a protocol for addressing this particular issue.

    It was particularly enlightening to witness the recognition of psychological aspects and their role in post-Bariatric care. The conference speakers delved into the psychological facets of Bariatric patients, exploring their behaviors and cognition. Understanding the psychology of Bariatric patients is indispensable for providing them with the appropriate support, and I gained valuable insights into various methods to assist them in their journey of weight loss and psychological adaptation.

    The discussions on acceptance-based adherence, especially in the context of physical exercise, illuminated the importance of holistic approaches to patient care. They also underscored the need for educational and skill training among dieticians to enhance patient adherence. The pivotal roles of patient engagement and motivation in their success were highlighted. Furthermore, I acquired knowledge about the impact of phonotype on sedentary lifestyles, physical activity, and sleep cycles. The intricate interplay and reciprocal relationship between these factors provided me with a more comprehensive understanding of how they collectively affect the lives of Bariatric patients.

    One aspect that resonated deeply with me, and which I intend to explore further, is the impact of food on weight regain. The conference revealed that a significant percentage of Bariatric patients taking psychiatric medications encounter challenges related to weight gain. It became evident that the difficulty in adhering to strict dietary guidelines after surgery is a major factor contributing to weight regain. Moreover, it was concerning to learn that a substantial portion of patients exhibit signs of eating disorders. This underscores the need for more research and comprehensive support to address these issues and assist Bariatric patients in a more holistic manner.

    In addition to the invaluable exchange of information and knowledge, the organization of the entire conference was truly captivating. However, there was one minor aspect that merits consideration for improvement, and that pertains to the late-night transportation. This, if I may offer a suggestion for future events, could be further enhanced.

    In conclusion, this conference served as a remarkable opportunity for me to expand my knowledge and deepen my understanding of the complexities surrounding Bariatric patients and the challenges they confront. It has ignited my passion to continue exploring this field, with a specific focus on the intricate relationship between food, psychiatric medications, and weight regain. I am enthusiastic about contributing to the enhancement of Bariatric patient care and support in the future.

    I am writing to express my sincere gratitude to you for making possible this Scholarship to attend the IFSO Naples Congress. I was thrilled to learn of my selection for this distinction, and I am deeply appreciative of your support. Also, you have given me one of the IFSO scholarship award with public recognition at the Gala Dinner and this gave me an enormous honor to me and my family as a representative of my country as an IFSO member.

    I've been working in general surgery at my country, Paraguay, and I’ve been also working in the field of bariatrics since. With this sholarship, I was able to continue learning in many topics as complex bariatric procedures, updates, new techniques and others relevant issues. In this sense, the financial assistance you provided me was a great help in order to increase my educational expenses, and it allowed me to concentrate more of my time on improve my knowlede and skills in bariatric and metabolic surgery.

    In addition to the academic experience, I had the opportunity to know the city of Naples and some of its tourist attractions, leaving me the best memories and the feeling that I will return again to this wonderful city, which welcomed me so well.

    I have nothing to suggest or complain about this great experience, everything was perfect. Thank you again for your generosity and support. I will work very hard and eventually give back all the learned to others surgeons like myself, my patients and my comunity to spread that the metabolic and bariatric surgery is the gold standard treatment to adiposity chronic disease and its metabolic disorders.

    First of all, I am sincerely thankful to the IFSO team and committee for the scholarship, as it was an eye opener to me to join the 3-day conference. I was able to meet, see in person and listen to talks and debates by the pioneers in the field of bariatric surgery, which was very inspiring to say the least. The presentations on novel operations and also the latest guidelines on bariatric surgery were a great exposure that will help me in my clinical day to day practice. It enhances my ability to offer patients quality care and treatment options.

    Furthermore, meeting other surgeons that share similar interests as myself, sharing ideas and learning about different practices across different continents was an extremely valuable experience. It showed me how differing clinical practices can be and the expert panels and talks further provided further pros and cons of different techniques and surgeries.

    All in all, it was an amazing and enjoyable experience and I have gained a lot of knowledge and also new friends. My immense gratitude and congratulations for the IFSO team for such a job well done. Looking forward to IFSO Melbourne 2024!

    IFSO Surgeon Scholarship Post Congress Report

    As a young Chinese bariatric surgeon, I am honored to receive the 2023 IFSO Surgeon Scholarship, which enable me to attend this global event at the IFSO headquarters in Naples, Italy. Due to the fact that COVID-19 has been ravaging the world for many years, we suspended our international academic exchanges for three years and were unable to share our experiences face-to-face with experts offline in order to gain the most up-to-date information. This year's sponsorship by IFSO for my participation in the conference has greatly increased my interest in the emerging field of bariatric surgery, and I plan to use my efforts to improve the multidisciplinary diagnosis and treatment of patients with obesity in China.

    I attended the postgraduate program on the first day of the conference. One of the goals of my participation was to study the theory and practice of endoscopic treatment of obesity and to introduce cutting-edge techniques to China. I spent the day attentively listening to each endoscopy session and was delighted to see that the ESG produced excellent results. I personally applied the ESG procedure in the afternoon, and many of the potential dangers I anticipated have been eliminated by the improved equipment. Our department is the first in China to perform a variety of endoscopic techniques for weight loss, including endoscopic botulinum toxin injection, endoscopic fundoplication, endoscopic diversion stent placement, and endoscopic duodenal mucosal destruction. However, endoscopic weight loss techniques have yet to be performed in significant numbers in China due to numerous limitations, such as a lack of health insurance coverage, a lack of devices, a lack of patient trust, and poor endoscopic surgical skills. This time, I also communicated with a number of foreign manufacturers to seek in-depth cooperation to introduce good devices into China and to increase the number of endoscopy training courses, so that more Chinese surgeons can be trained in endoscopy and assist their patients in losing weight through minimally invasive techniques.

    I was completely immersed in the conference's academic feast for the next two days. Sleeve gastrectomy accounts for close to 90% of cases in China, surpassing all other surgical procedures by a significant margin. This is mainly due to the fact that it is safer and easier, resulting in fewer surgical and nutritional complications. As a result, sleeve gastrectomy is frequently exploited in our country, leading to a great deal of inadequate weight loss, weight regain, and unrelieved comorbidities. Our department adheres to a certain percentage of RYGB, which provides additional benefit, particularly in relatively elderly patients with a longer duration of diabetes and diminished islet function. Therefore, I am very interested in long-term follow-up studies, comparative operative studies, and surgical safety studies of all bariatric procedures presented at the Congress. There were two studies that impressed me the most. The first is a multicenter study of Side-to-side Linear Magnetic Duodenal-ileostomy for the treatment of obesity with or without T2DM. This innovative approach to surgery caught my eye, and I felt the surgeon's clever brain and boundless imagination. The second study was a prospective, randomized, controlled comparison of OAGB and RYGB. In recent years, OAGB has become more prevalent in Chinese bariatric surgery; it is considered safer than RYGB, but can achieve comparable weight loss and glucose lowering efficacy. However, we are still concerned about the incidence of surgical and postoperative nutritional complications in OAGB; after decades of validation, RYGB has also become the gold standard surgical procedure, but OAGB lacks long-term follow-up cohorts. The results of this study indicate that OAGB has comparable weight loss and glucose-lowering effects to RYGB, which is still encouraging. However, there is a nearly 2-fold incidence of GERD and unexplained hypoproteinemia, which affects the quality of patient survival, and 8% of OAGB revisions to RYGB. It suggests that OAGB ought to be administered with caution, and we will conduct a long-term follow-up study to provide Chinese data for international use.

    I was ecstatic to see a number of Chinese teams attending the conference and giving academic presentations, which demonstrates that while our surgeons are striving to increase surgical volume, they are also working diligently to summarize their own experiences and present our results and experiences through academic research. In addition, over ten Chinese companies attended and sponsored the conference, working in the field of weight reduction to provide surgeons with more effective tools.

    Naples is a passionate and romantic city and the farewell dinner was held at the beautiful Bagno Elena Beach Club. Experts from all over the world gathered on the beach, enjoying the sea breeze, wine and music, and talking about the future of bariatric surgery. A wonderfully unforgettable experience I must appreciate IFSO for their encouragement and support. I will keep track of IFSO's training programs and courses for young surgeons, and I will strive to improve medical services for obese patients in China and around the world. I aim to become a member of the Young IFSO family and contribute my talents. Thanks again to IFSO!

    There are two small suggestions. Firstly, in the future, it would be better to set the official hotel of the conference closer to the place where the conference will be held, so as to make it easier for participants to travel. Secondly, we hope that the photos of the conference and the participants taken by the official staff can be posted on the official website or other platforms, so that the participants can keep their memories, thank you.

    I participated in the XXVI IFSO NAPLES 2023 World Congress. I had received a scholarship from IFSO worth US$ 2,500 that gave me the opportunity to participate in this important congress. In it, I was able to talk about the work that I do in the Public Health System in Brazil; exchange experiences and knowledge with professionals from many countries; and added knowledge to my long career in researching and caring for Bariatric surgery patients.

    Additionally, I participated in the discussion on "Barriers to Contain the Recurrence of Weight", held in the Postgraduate Course on "The Importance of Multidisciplinary Care in Optimizing the Results of Bariatric and Metabolic Surgery", which was coordinated by Dr. Mary O' Kane. In the Physical Activity - The Whole on Day Matters session - I joined as a moderator and gave the talk, "Supervised Exercise: How and Why". Furthermore, I could forward 6 E-books to be presented at the congress, according to the titles below:

    I started my career soon after completing my education, and the initial five years were only a struggle to establish the recognition of my subject.

    Then, in 2008, i got a chance to contribute to AIIMS, New Delhi Bariatric Surgery Program. I gave heart and soul to my bariatric responsibility. Bariatric surgeons in My country recognized my potential. They gave me the more significant task of conducting academic activities from the platform of the Obesity and Metabolic Surgery Society of India (OSSI). We completed the endorsed course Certificate for Integrated Care of Obesity ( CICO) in 2023. This effort was well appreciated and recognized by IFSO 2023 in Naples. They awarded me with the IFSO-IH Scholarship, which gave me the freedom to present my work to the international community working in the field of Obesity and Metabolic Surgery.

    I got a chance to explore Naples and learn about their culture. Thank you, IFSO 2023, for outstanding academic and cultural events and knowledge exchange.
